LUTH Community Health Officers Course Admission Form 2025
LUTH is now accepting applications for its two-year Community Health Officers (CHO) residential programme for 2025/2026. If you have the right experience and licence in community health, you can apply online. Read on for entry rules, steps to apply, and key deadlines.
The Community Health Officers (CHO) School at the Lagos University Teaching Hospital (LUTH) is now open for applications for its two-year Higher Diploma in Community Health for the 2025/2026 academic session.
This residential programme takes place at the LUTH Primary Health Care Complex in Pakoto and the LUTH main campus at Idi-Araba, Lagos. It is fully approved by the Community Health Practitioners Registration Board of Nigeria (CHPRBN).

Meet the requirements for CHO programme admission
You can apply for the CHO programme if you meet both of the following:
- Hold a Diploma in Community Health with at least 2 years of work experience, or
- Hold a Community Health Extension Workers Certificate with 5 years of work experience
Also, you must:
- Be registered with CHPRBN
- Have a valid practising licence
These conditions help ensure that all candidates have enough background and field experience in community health work.
Follow these steps to apply for LUTH CHO admission
The application is online. Here is the full guide:
- Visit the official course portal: luth.gov.ng/courses
- Select School of Community Health from the list
- Fill in the Account Creation Form with correct details and an active email address
- Check your email for login details
- Click on Make Payment to go to Remita
- Use your RRR code to pay the ₦10,000 application fee
- After payment, log in at luth.gov.ng/profile
- Complete and submit the application form for the CHO course
- Print out your application bio-data for the entrance screening
Apply early before the deadline closes
The deadline to apply is 12 weeks from the publication date. Submitting your form early gives you enough time to prepare for the next stage.
