UCH Ibadan HND Admission Form 2025/2026

The Higher National Diploma (HND) in Health Information Management at UCH Ibadan is a two-year non-residential course starting in September 2025. You can apply if you meet these requirements:

• Hold a National Diploma (ND) in Health Information Management
• Have a MERIT or PASS grade in your ND
• Own an ND practising licence, or have at least 3 years of work experience in the field
• Be medically fit and not pregnant during the training

You must also be prepared to present original certificates at every stage.

Prepare for the computer-based test and interview

If your application is accepted, you will sit a Computer-Based Test (CBT) covering:

• Health Records Management
• Anatomy and Physiology
• Clinical Coding
• Biostatistics
• Computer Science
• Health Planning Management

If you pass the CBT, you will be invited for an oral interview. All dates and updates will be sent by email or posted on the school’s website.

Follow the steps to apply online

You can start your application from Monday, 28 April 2025 on the UCH Schools Portal. Follow these steps:

1. Go to the portal and click on REMITA PAYMENT
2. Make the payment
3. Sign up using your RRR, email, and password
4. Log in and download the Applicants User Guide
5. Fill in your details and complete the application
6. Print 2 copies of your completed form and photocard
7. Bring both to the exam centre on your test day
8. Make a separate payment of ₦500 to the Health Information Managers Association of Nigeria:
   • Bank: Union Bank, UCH Branch
   • Account Number: 0000207307

Only complete applications with proof of payment will be accepted.
