- JAMB admission letter (institution copy)
- Application for admission form (original and photocopy) – for the Admissions Office only
- Acceptance form
- Student clearance form
- Medical fitness report
- Student personal data form
- Anti-social activities form
- Security check form
- Original JAMB result slip
- Original and photocopy of O’Level certificates (WAEC, NECO, or NABTEB)
- Original and photocopy of birth certificate or statutory declaration of age
- Local Government identification letter
- Attestation letter addressed to the Registrar
- Payment permit
- Photocopies of all payment receipts and bank tellers
- Two recent passport photographs
